TMJ Dysfunction in Reno, NV & Surrounding Areas Expert Explains TMJ Dysfunction

When I first moved to Nevada in 1982, I was the new Orthodontist in town from New York City and had just completed my Orthodontic residency at New York University the year before. TMJ issues were not widely covered in our program and that seems to be true for most even to this day. We were told that if patients had a painful, clicking jaw, send them to the oral surgeon for treatment because we had very little to offer our patients with TMJ dysfunction in Reno, NV, and surrounding areas.

Going Against the Status Quo

I became alarmed when the patients I referred to the oral surgeon came back with their TMJ meniscuses (disc) removed and replaced with Teflon Protoplast artificial discs. I had several friends who were orthopedic surgeons and I learned from them that this was tried with knee meniscuses with very bad consequences. The Teflon Protoplast implants caused a giant cell macrophage attack from the immune system due to the fact that they were a foreign body and they had to be removed after becoming painful and limiting range of motion. I brought this up at our local Northern Nevada meeting and was basically told that I was out of order telling an oral surgeon how to practice and should sit down and shut up. “Get a rope, He’s from New York City.”

Seeking Answers

I stopped referring patients to oral surgeons and started my own journey to learn as much as possible about treating patients with TMJ problems. From Myotronics in Seattle to the University of San Francisco pain clinic, Rocobado Physical Therapy courses, to John Barnes Myofascial Release techniques, I became a seeker of answers to my patients’ troubles. Some time by the late 1980’s all patients who had those Protoplast Implants placed suffered the exact giant cell macrophage reactions and had to have the implants removed. Hate to say I told you so, but…

Evolving Solutions

My treatment has evolved over the years, always adding or subtracting what is working or not. The most important part of the puzzle is an accurate diagnosis. A complete examination with a 3D cat scan of the TMJ allows me to see what we are dealing with. A T-Scan computer-aided bite evaluation is critical to know if a bite problem exists. Patient history of trauma, a blow to the jaw, a motor vehicle accident, chronic clenching and grinding, etc, are critical to understanding the puzzle. My treatment is not cookie cutter. It can be as simple as a bite plane to prevent nighttime clenching and grinding. It becomes more involved depending on the extent of headaches and clicking and popping of the jaw. Interestingly enough, a recently formed company called Trudenta uses very similar treatment protocols that I do, but they charge over $80,000 for their consulting services and have dentists billing $7,000-$9000 for their treatment. Our fees are substantially lower. I cannot recall the last patient who had to have jaw surgery for their TMJ issue in my office.

Invisalign First® in Nevada

It’s hard to believe that just one year ago, my lovely granddaughter Ashlynn became my first patient for Invisalign First in Nevada. Ashlynn lives in Austin, Texas and I knew that treatment for her bite problem was going to be difficult with traditional braces. Bonding is a palatal expander and having her Mom turn the key on the ‘Mouse Trap” was not going to score any Grampa Points. Invisalign had just launched a new design for Aligners that are made specifically to fit over both baby teeth and permanent teeth with the ability to expand the arches without the need for the bulky and difficult expanders of the past. They call it Invisalign First and I can now treat my younger patients, age 7-9, with a kinder and gentler approach to making room for crowded teeth and fixing bites.

Treating Young Patients

Ashlynn had an underbite that she inherited from her Dad, I noticed it from the time she was three years old and always had a plan on how to create a beautiful smile for her. Thousands of young patients have been treated by me with varying orthodontic devices to correct serious bite problems over my 37 years of practice and I’ve always worked to find a way to make it more tolerable. Progress was always slow but relentless. I have been to every Annual Scientific Meeting of the American Association of Orthodontists, save one since I was a resident. I was always searching for that new approach that would make our treatment more comfortable and still get excellent results.

One Happy Patient

When Align Technologies announced Invisalign First, I knew Ashlynn would be patient number one for me. She was a bit disappointed about not having braces because she thought the colors were cool. I showed her that we could put a sticker on the Aligners and she felt that was even better because she changes to a new one each week. One year later, her treatment has gone very well. Her underbite is completely corrected and her smile is so wide and bright she lights up a room when she enters.

The Benefits of Early Treatment

I did this with the help of a wonderful colleague in Austin, TX and a very cooperative patient who now has the greatest confidence in her smile. I am a big proponent of Early Phase 1 Treatment where it is appropriate and necessary. The American Association of Orthodontics recommends a child’s first evaluation by age seven. If I find something that should be addressed now I will explain the reasons for early treatment and they have to make sense to the parents and the patient. Most times we have our young patients come back in 6 or 9 months for a growth evaluation. Invisalign® Provider in Reno, NV & Surrounding Areas Discusses Complex Invisalign Cases

Almost every day, I meet patients who have been told that they could not have Invisalign because their problem is too complex, so they must wear braces. I was the first certified Invisalign provider in Reno, NV and surrounding areas almost 20 years ago. Back then, I was not very confident in the early versions of Align Technology and used it for patients with mild to moderate crowding and little else. If a person had an open bite (the front teeth can’t bite through foods like pizza, sandwiches, etc), we did not believe it would be possible to close those teeth together with plastic aligners. Fast forward 10 years later, and we discovered that Invisalign treats open bites way better than braces–and the final result is more stable.

The Combination Effect

When it comes to severe bad bites like underbites (lower front teeth are out in front of the uppers) we use a combination of Invisalign and a Motion Appliance. This is a very new concept in Orthodontics that is called ”Fix the Bite First”. Dr. Luis Carriere, a brilliant Spanish orthodontist, is the inventor of the Motion Appliance and the pioneer of this new “bite first fixing” that we do. The hardest part of orthodontics is getting your bite right and doing that first cuts treatment time down significantly. Patients have been told that they need jaw surgery and I have treated them without surgery with Invisalign – and actually fixed their bite.

The Mandibular Advancement Feature

In November of 2018, Invisalign received FDA approval for their Mandibular Advancement feature that allows us to correct patients with very profound buck teeth by moving their lower jaw forward and their upper teeth back. I personally had some input in the design of this new feature. It mimics what we have been able to do with Herbst and MARA orthopedic appliances without the bulk and discomfort of those heavy metal devices. The Mandibular Advancement feature has been approved in Europe and Canada for the last four years and over 14,000 patients have been treated. Our results so far have exceeded my expectations. We are seeing our young patients blossom as their faces change and their airway is opened up so they can get more oxygen while they sleep.

Orthodontic Specialist in South Lake Tahoe, CA & Surrounding Areas – The Differences In Orthodontic Treatment for Girls & Boys

“Snips and snails and puppy dog tails, that’s what little boys are made of. Sugar and spice and everything nice, that’s what little girls are made of.” As an orthodontic specialist in South Lake Tahoe, CA and surrounding areas, that that old 19th-century nursery rhyme has us all aware that boys and girls are quite different. In my world of Orthodontics, that difference is most apparent in the area of growth and development. As in life, timing is everything. Successful outcomes depend on timing and it is especially important to see how these girl vs. boy differences show up in orthodontic treatment.

The Maturity Factor

As a general rule, girls mature faster and their pubertal growth spurt ends earlier than boys. In other words, by the time a young lady reaches 14 years old, 90% of her facial growth and development is complete. Young lads will continue to grow until their early 20’s. This becomes critical when my patients have jaws that are not growing enough (an overbite), growing too much (an underbite), or have mismatched upper and lower jaws that result in a crossbite.

Once Again, Timing is Everything

The critical thing is timing. Girls have a shorter window for effective non-surgical jaw corrections, so it is more important to get those young ladies in for an orthodontic evaluation by age seven. Boys can always benefit from an orthodontic evaluation at age seven, but they have a longer window for effective dento-orthopedic treatment.

Best Carson City, NV & Surrounding Areas Orthodontist Explains Extractions & Surgery

There has always been an ongoing debate among the best Carson City, NV and surrounding areas orthodontists about the need for extracting permanent teeth as a part of orthodontic treatment. It seems the Orthodontic residency one attends has a big influence on treatment philosophy. I had exposure to both sides of the debate at NYU and I landed on the side of rarely extracting teeth as my treatment of choice.

Effects of Extractions

One of the biggest reasons is the effect it has on the face and the patient’s airway. Think of it in terms of the garage where you park your car. It fits but not with much extra room to close the garage door. Now imagine you took out four building blocks, one on each side, and went to park your car. It would stick out of the garage door. Much the same happens when we extract permanent teeth. Your mouth shape (the garage) becomes smaller and your tongue (the car) sticks out into your airway space.

Negatives & Positives

I‘ve had an ENT doctor mention that he sees more snoring and sleep apnea problems in patients that have had teeth extracted. That said, there are times when there is just not any room and no realistic way to create space for all the teeth to fit. My own practice statistics put my extraction rate at 7%. That is much lower than our professions average of 20-45%. Facial aesthetics is also affected by extracting teeth. Plastic surgeons are almost always filling up spaces in the face for a more youthful appearance. Having a nice wide arch with all your teeth creates a beautiful smile design for the majority of people.

Questions to Ask Before Surgery

When it comes down to patients needing jaw (Orthognathic) surgery to correct a skeletal bad bite, three questions must be asked.

1) Is the patient willing to have the surgery performed?

2) Will the surgery achieve a significant improvement in facial appearance and dental function?

3) Are the financial resources there, with or without medical insurance coverage to pay for the attendant costs of the surgery?

If the answer to all three is ‘yes’ then working with a skilled and experienced Oral and Maxillofacial surgeon allows me to achieve excellent patient outcomes. A ‘no’ answer to any of these questions leaves patients stranded with few options. I have treated many patients who have been told that surgery was their only option. The majority of these patients have been happily treated with my Specialist-directed Invisalign therapy.
